I think the end of my NA Steering rack popped out

I think the shudder when braking might be caused by this. (Looking under the rubber steering boot on driver's side )
There is what looks like a loose washer on the rod from steering rack. Am I right in assuming it has popped out of the end of the steering rack? How can I get it back in the spot it should be?

Another picture. Looking from front of car, driver's side

This photo is upside down, it is the driver's side.
The ball joint in middle also has some play, so that is also contributing to the shudder under braking.

Nope, that's normal. Rubber faced bump stop. Shudder only under braking will be warped front rotors. Worn out tie rod ends won't help anything either.

OK, thanks. That means that floating washer should have rubber attached to one side of it. I looked at the manual and other images but could not find any reason for that washer.

Shudder under braking is far more likely to be attributed to worn/warped/uneven Brake Rotors(this can contribute to the displacement of the spacer in your pics).
